Out of Network Claim

9FIVE is an out-of-network provider. When working with an out-of-network provider, most insurance companies will reimburse purchases related to eyewear and prescription services. When using out-of-network coverage you will need to purchase the product on your own, in full. Next a claim can be sent to your insurance provider for reimbursement (typically by check).

1. Add product(s) to "Cart"

2. Click "Cart"

3. Choose "PayPal" as payment option
Select PayPal

4. Choose "Pay with Debit or Credit Card" and continue as guest even if you have a PayPal account
Select Debit or CC Payment

5. Fill out all required information, including your HSA/FSA payment information
Fill out fields

6. After finishing step 5, you will be redirected back to 9FIVE.com. Proceed to normal checkout on 9FIVE.com
