We recommend that you check with your insurance provider for your out-of-network benefits eligibility before purchasing. We can submit your claim if you currently are insured by the following companies:

Once we receive your insurance information, a confirmation email will be sent to you shortly.
 
If your insurance company is not listed above, you can request an itemized receipt from us and file a self-claim directlymthrough your provider. You can make this request by contacting rx@9FIVE.com and providing us your full name and order number.
