Because we are an out-of-network provider, you will need to purchase the prescription service/frames upfront, however 9FIVE can submit an insurance claim directly to your insurance provider. We recommend that you check with your insurance provider for your out-of-network benefits eligibility before purchasing. We can submit your claim if you currently are insured by the following companies:

- Spectera
After you completely checkout with us, please contact rx@9FIVE.com and include the following information in the body of your email.

9) 9FIVE Order Number
10) Date of Birth
Once we receive your insurance information, a confirmation email will be sent to you shortly.
If your insurance company is not listed above, you can request an itemized receipt from us and file a self-claim directlymthrough your provider. You can make this request by contacting rx@9FIVE.com and providing us your full name and order number.
